New Patient Registration

To register as a new patient at Broadway Health Centre, please follow these simple steps:

  • Download the registration form from our website and complete it.
  • Bring the completed form, along with your photo ID and proof of address, to our reception desk.
  • If you are registering a child under five years old or if you are a career, please complete the additional forms available on our website.
  • After registration, we will conduct a new patient check-up to ensure you stay healthy and happy.

Sharing Your Medical Record

Increasingly, patient medical data is shared, e.g. between GP surgeries and District Nursing, to give clinicians access to the most up-to-date information when attending patients.

Our systems must obtain patients’ consent before sharing any of their medical information. Patients are required to provide their approval for sharing their data with other health providers, and they must also specify which providers can access their data.

Sharing patient data is necessary but doesn’t have to be shared with everyone. Patients can allow data sharing with specific departments while disallowing access to others. By doing so, patients can control who has access to their data.

Change of Address

The practice must have your current address and telephone number in case of emergencies or routine matters. If your address or telephone number has changed, please inform the practice in writing as soon as possible. You can do this by filling out a simple form at the surgery’s reception desk.

Please note if you are notifying us about a change of name, you must visit the surgery with supporting legal documentation to support this.​

Summary Care Record

The Summary Care Record (SCR) is a new NHS computer system that provides emergency doctors and nurses with vital medical information about a patient, especially when the surgery is closed. Initially, it contains only medication and allergy information. As the system develops, other NHS staff members will have access to it, too. It includes data from hospitals, out-of-hours services, and specialists’ letters. The SCR aims to improve emergency care.
Your information will be extracted from practices such as ours and held on central NHS databases.

Like any new system, there are advantages and disadvantages. When you speak to an emergency doctor, you might miss out on something important, and having access to your medical records could prevent mistakes or problems. However, even in such cases, NHS staff must ask for your consent whenever they want to access your records unless you are medically incapable.

If you’re hesitant about sharing your personal information, you can choose to keep it within this practice. The Summary Care Record, managed by Connecting for Health (CfH), has agreed with doctors’ leaders that new patients can decide whether or not their information is uploaded to the Central NHS Computer System.

For existing patients it is different in that it is assumed that you want your record uploaded to the Central NHS Computer System unless you actively opt out.

Welcome to the Patient Participation Group (PPG) at Broadway Health Centre!

The Goal of the PPG

PPG is a group of volunteer patients who work with their healthcare providers to improve local healthcare services.

  • To offer opinions constructively and to put forward ideas on behalf of other patients.
  • To improve the provision of health care.
  • To improve communication between surgery, patients and the broader community about matters concerning the surgery and health in general.
  • To assist in the development of new services.
  • To encourage a spirit of self-help and support amongst patients to improve their health and social care.

All meetings are held on the First Monday of each month from 12:00 – 13:00
